Trip Forms
In our ongoing effort to better serve our valued guests and to help protect our environment by reducing paper waste, we have posted our trip forms on our website. For improved security and usability, we ask that clients use our new Trip Forms by visiting forms.nathab.com. There, you will be asked to login using your booking-specific Username and Password. Should there be complications that prohibit you from using the new forms, you may use our Legacy Trip Forms which do not require a login. The following Legacy Trip Forms can be found in this section:
Booking Form
This form (one per two travelers) must be submitted within 14 days of booking your adventure. To access your Booking Form, click here.
Terms and Conditions
All travelers must complete this form prior to travel. Your reservation is not confirmed until you've completed this form. To access Terms and Conditions, click here.
Insurance Form
This form must be submitted to either purchase or decline travel insurance. Insurance must be purchased within 14 days of your booking date in order to be covered for any preexisting conditions or to take advantage of our "Cancel for Any Reason" waiver. Luggage Restrictions
